2) Applicant did not fill out state fire marshal inspection 3b) Two toy shelves were within 3 ft of fireplace 3f) Applicant has fireplace and does not have written statement declaring they would not use it at any time 3g) Applicant did not have carbon monoxide detector 3i) Fire extinguisher was not located per state building codes and top of fire extinguisher was 5'11" off ground (more than 5 feet off floor)
